2. Most recent membership list, including contact information inclusive of name, address, telephone, and email of all members.

This information is acceptable in whatever format available, including electronic format. Please be advised this our second request, the prior request having been made to Mr. Whitworth in our meeting with him in 2004, which request went unanswered.
